Things you should Know

THINGS YOU SHOULD KNOW BEFORE HAVING SPINAL SURGERY

1.  Did you know that 39% of spinal surgery recipients do not have even minimal improvement? 

This is called Failed Back Surgery Syndrome.

What is Failed Back Surgery Syndorme (FBSS)?

      Following open spine surgery, whether it is a fusion, laminectomy, or a discectomy patients can experience chronic pain in the back and or the arms or legs. These syndromes are often due to identifiable causes but are termed by surgeons and the medical community to be failed back surgery syndrome (FBSS). Nearly all open back surgeries include one form of fusion. The true test of whether an open back surgery is successful is not to measure the success of the fusion process but the actual reduction of pain. Although most published articles describe an 80% good/excellent result rate, personal experience by most physicians in the field suggests that the true proportion of good to excellent results is closer to 50% for single level fusions, while with three levels, this drops to approximately 15%.

The most common causes of FBSS do not involve technical errors on the part of the surgeon, but are due to the inherent risks of the body's reaction to an invasive procedure. Some of the most common causes include:

  • Epidural fibrosis, or scarring of the nerve roots, is a common cause of FBSS and it involves no technical errors on the part of the surgeon. It is simply an unfortunate response of the body to the trauma of surgery.
  • Other identifiable causes of the FBSS include inadequate decompression which results in untreated lateral recess and foraminal stenosis.
  • Residual foraminal stenosis.
  • Residual spinal stenosis.
  • Painful disc disease due to residual pain emanating from the discs which still retain motion.
  • Spinal instability due to removal of too much of the central disc or supporting ligaments.

2.  If you are not committed to follow the isometric exercise program, do not have decompression therapy.

Decompression therapy is a very powerful tool, highly recommended and, when appropriately used, can help disc patients who have experienced failure in all other systems, including traditional chiropractic.

     However; Spinal Decompression just like other treatments, including Spinal Surgery, Pain Medications and injections, is a treatment that does not address the cause of the problem. The reality is that decompression without proper muscle strengthening and rehabilitation is actually just a very expensive Band-Aid.

     Many patients who have spent a lot of money for decompression therapy have had their symptoms return within three to six months.  However, it is the rehab that creates the stability and correction to enhance and stabilize the benefits of spinal decompression. So it is very important not to miss this step.

     When it comes to rehabilitating the spine, certain specific factors must be considered. First of all, traditional, conventional rehab, which involves performing resistance exercises through a range of motion (i.e., weights, machines, therabands) along with electrotherapeutic machines (i.e., electric muscle stimulation) do not address this problem. The reason is that they are working the fast-twitch muscle fibers. Fast twitch muscles will make you strong, but will not change the structure of the spine or remove the underlying cause of the disc injury. They will help you to carry your groceries, while the spinal structure continues to degenerate.

     The muscles that support spinal structure and determine spinal stability, the shape of your spinal curves and the future of the health of your spine; are postural, slow-twitch muscles. Slow-twitch muscles that support spinal structure require isometric exercises and are very different from conventional therapy.

With our decompression protocol, you will be given a biofeedback sphygmomanometer and we will train you in its use in order to have you perform your isometric exercises properly. Consistency is the key.  You will get started on these exercises as soon as you are out of pain and you must continue performing your exercises after your treatment is completed (you perform these exercises lying down, they are simple and don't take much time at all).  Chiropractic adjustments will also help in this process.

3.  We will do what is best for you.

There are times when surgery is the only and best option.  A ruptured disk is one of these conditions.  We will evaluate your condition and tell you if you are a good candidate.  The true test of whether decompression is working for you is the reduction of pain.  If after 8 decompression treatments you do not show noticeable improvement we will  further evaluate your condition and order any necessary tests such as MRI scans, and arrange  for a surgical consultation.
